Одной из основных целей при лечении тромбофлебита подкожных вен является разобщение поверхностных вен от глубоких в местах наиболее частого перехода тромботических масс. В то же время выполнение методов эндовенозного лечения при тромбофлебите не имеет доказательной базы, регламентирующей все аспекты выполнения данной процедуры.</p></sec><sec><title>Цель</title><p>Цель: повышение эффективности эндовенозной лазерной коагуляции у пациентов с варикозной болезнью, осложненной стихшим тромбофлебитом подкожных вен. Для этого в ООО «Башмедцентр» была разработана и внедрена в клиническую практику модифицированная методика эндовенозной лазерной коагуляции большой подкожной вены (патент № 2827090 C1).</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. На базе ООО «Башмедцентр» с 01.01.2022 по 31.12.2022 у 50 пациентов, имеющих варикозную болезнь нижних конечностей и стихший тромбофлебит большой подкожной вены умеренного риска, была выполнена эндовенозная лазерная коагуляция. Обследуемые были разделены на контрольную группу (34 пациента) и группу сравнения (16 пациентов). У всех пациентов выполнялась оценка качества жизни до и после оперативного вмешательства с использованием визуально-аналоговой шкалы (ВАШ). Также была выполнена первичная экономическая оценка стоимости оперативного вмешательства у пациентов со стандартной и модифицированной методиками эндовенозной лазерной коагуляции.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. До операции статистически значимой разницы качества жизни не было между обеими группами. В группе модифицированной методики эндовенозной лазерной коагуляции получены положительные клинические и эстетические результаты после операции. Была выполнена экономическая оценка затрат на оперативное лечение, а также измерение продолжительности этапа лазерной коагуляции, показавшие преимущество модифицированной методики. Пациенты, перенесшие модифицированную методику эндовенозной лазерной коагуляции, показали статистически значимо лучший уровень качества жизни на контрольных осмотрах на 7, 30 и 90-е сутки после выполненной операции.</p></sec><sec><title>Заключение</title><p>Заключение. Данные, полученные в ходе сравнительного анализа результатов лечения, показали эффективность модифицированной методики эндовенозной лазерной коагуляции большой подкожной вены у пациентов с варикозной болезнью и стихшим тромбофлебитом умеренного риска, что проявляется в виде более высокого уровня качества жизни, лучшего клинического результата и меньшей длительности операции.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. One of the main goals in the treatment of thrombophlebitis of saphenous veins is to separate the superficial veins from the deep ones at the sites of the most frequent transition of thrombotic masses. At the same time, the implementation of endovenous treatment methods for thrombophlebitis lacks an evidence base regulating all aspects of this procedure. Aim. This study aims to increase the effectiveness of endovenous laser coagulation for patients with varicose complicated by declined thrombophlebitis of subcutaneous veins. To that end, a modified technique of endovenous laser coagulation of the great saphenous vein was developed and implemented in clinical practice at Bashmedcenter LLC. This modified surgical intervention aims to reduce the economic costs of performing the procedure, shorten surgery duration, and improve the cosmetic result and the quality of patients’ lives. The procedure of the operation was patented, and the patent for invention No. RU 2827090 C1 was issued.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. From 01.01.2022 to 31.12.2022, 50 patients with varicose of the lower extremities and moderate-risk thrombophlebitis of the great saphenous vein underwent endovenous laser coagulation in Bashmedcenter LLC. The examined patients were divided into a control group (34 patients) and a comparison group (16 patients). All patients underwent an assessment of the quality of life before and after surgery using a visual analog scale (VAS). A primary economic assessment of the costs for surgical intervention in patients with standard and modified endovenous laser coagulations was also performed.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. Before surgery, no statistically significant difference in quality of life between the two groups was observed. After surgery, positive clinical and aesthetic results were obtained in the group of modified endovenous laser coagulation. An economic evaluation of the costs for surgical treatment and measurement of the laser-coagulation duration were performed, which showed the advantage of the modified technique. According to statistics, patients who underwent a modified technique of endovenous laser coagulation of the great saphenous vein showed a significantly better quality of life at follow-up examinations on days 7, 30, and 90 after surgery.</p></sec><sec><title>Conclusion</title><p>Conclusion.
