Гастропанкреатодуоденальная резекция является наиболее трудной операцией в абдоминальной хирургии. Развитие современной хирургии сопряжено с появлением малоинвазивных методик операций, которые выполняются с использованием лапароскопических и роботических технологий. При этом роботические технологии имеют ряд технических преимуществ по  сравнению с  традиционной лапароскопией, которые могут иметь большое значение при операциях на поджелудочной железе. Цель исследования: провести анализ эффективности и безопасности робот-ассистированной гастропанкреатодуоденальной резекции.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Настоящее исследование было проведено на  базе хирургического отделения Клиники БГМУ за  период с сентября 2020 по сентябрь 2025 года. Для анализа результатов оперативного лечения были отобраны 27 пациентов. При этом 1‑ю группу составили 12 пациентов, которым была выполнена робот-ассистированная гастропанкреатодуоденальная резекция, а во 2‑ю группу вошли 15 пациентов, которым была выполнена открытая операция. Анализу подвергались периоперационные хирургические критерии, а  также ранние и  поздние послеоперационные осложнения в  сроки, стратифицированные согласно системе Clavien—Dindo. Статистическая обработка данных проводилась с использованием пакетов программного обеспечения Microsoft Office 2024 и SPSS (IBM SPSS Statistics 27). Уровень статистической значимости принимался достоверным при значениях p &lt; 0,05.</p></sec><sec><title>Результаты и  обсуждение</title><p>Результаты и  обсуждение. Обе группы пациентов не  имели статистически значимых отличий по показателям на дооперационном этапе (p &gt; 0,05). По интраоперационным показателям в  1‑й группе статистически значимо меньше объем кровопотери (p &lt; 0,001), однако вмешательства проводились дольше, чем во 2‑й группе (p &lt; 0,001). По послеоперационным показателям в 1‑й группе, в сравнении со 2‑й, меньше сроки восстановления пассажа кишечника (p &lt; 0,001) и госпитализации (p = 0,004). По показателям 30‑дневных послеоперационных осложнений во 2‑й группе наблюдается их увеличение (46,7 %), однако статистически значимой разницы с  1‑й группой не обнаружено (p = 0,419). Робот-ассистированное вмешательство обладает целым рядом преимуществ, обусловленных минимизацией операционной травмы и прецизионностью манипуляций на  органах и  тканях.</p></sec><sec><title>Заключение</title><p>Заключение. Робот-ассистированная гастропанкреатодуоденальная резекция является эффективным и безопасным методом радикального лечения образований панкреатодуоденальной области.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Pancreatoduodenectomy is among the most technically demanding procedures in abdominal surgery. Recent developments in minimally invasive techniques, particularly laparoscopic and robotic systems, have reshaped operative strategies. Robotic systems offer several technical advantages over conventional laparoscopy, which may be especially advantageous in pancreatic surgery. Aim. This study evaluated the effectiveness and safety of robot‑assisted pancreatoduodenectomy.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. This study was conducted in the surgical department of the BSMU Clinic from September 2020 to September 2025. 27 patients undergoing operative treatment were included. Group 1 comprised 12 patients who underwent robot‑assisted pancreatoduodenectomy; Group 2 included 15 patients who underwent an open procedure. Perioperative outcomes and early and late postoperative complications were assessed and stratified according to the Clavien–Dindo classification. Statistical analysis was performed using Microsoft Office 2024 and IBM SPSS Statistics 27, with statistical significance set at p &lt; 0.05.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. Preoperative characteristics exhibited no significant difference between groups (p &gt; 0.05). Intraoperatively, Group 1 had significantly lower blood loss (p &lt; 0.001), although operative time was longer than in Group 2 (p &lt; 0.001). Postoperatively, Group 1 demonstrated faster recovery of bowel function (p &lt; 0.001) and a shorter hospital stay (p = 0.004). Although thirty‑day postoperative complications occurred more frequently in Group 2 (46.7%), the difference between groups was statistically insignificant (p = 0.419). The advantages of the robotic approach are attributed to reduced operative trauma and greater precision during dissection and reconstruction.</p></sec><sec><title>Conclusion</title><p>Conclusion.
