Рак молочной железы (РМЖ) остается ведущей причиной онкологической заболеваемости и  смертности среди женщин. Для гормончувствительного HER2‑негативного подтипа достижение полного патоморфологического ответа (pCR) после неоадъювантной терапии является ключевым прогностическим фактором. Цель: оценить эффективность и безопасность добавления тамоксифена к стандартной неоадъювантной химиотерапии у пациенток с гормончувствительным HER2‑негативным РМЖ.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В проспективное рандомизированное исследование включены 40 пациенток (пре- и постменопауза) с показаниями к неоадъювантной химиотерапии по схеме 4ddAC — 4T. Пациентки распределены на две группы: группа А (n = 20) дополнительно получала тамоксифен, группа В (n = 20) — только химиотерапию. Основной критерий эффективности — частота pCR, оцененная по шкале Residual Cancer Burden (RCB).</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. Частота pCR (RCB‑0) составила 10 % в обеих группах. Распределение остаточной опухолевой нагрузки по классам RCB было сходным, статистически значимых различий не выявлено (p &gt; 0,05). Комбинированная терапия продемонстрировала удовлетворительную переносимость, серьезных нежелательных явлений не зарегистрировано, все пациентки завершили лечение и были успешно прооперированы. Добавление гормональной терапии не дало значимого преимущества в достижении pCR, что подтверждает ограниченную роль тамоксифена в данном режиме.</p></sec><sec><title>Заключение</title><p>Заключение. Исследование показало сопоставимую эффективность и безопасность обеих схем лечения. Отсутствие преимуществ от комбинации с тамоксифеном подчеркивает необходимость поиска новых терапевтических стратегий для улучшения исходов у данной категории пациенток.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Breast cancer remains the leading cause of cancer morbidity and mortality among women. In hormone-receptor-positive, HER2-negative disease, achieving a pathologic complete response (pCR) after neoadjuvant therapy is an important prognostic factor. Aim. This study evaluates the efficacy and safety of adding tamoxifen to standard neoadjuvant chemotherapy in patients with hormone-sensitive, HER2-negative breast cancer.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A prospective randomized study included 40 preand postmenopausal patients with indications for neoadjuvant chemotherapy using the 4ddAC → 4T regimen. Patients were assigned to two groups: Group A (n = 20) received additional tamoxifen, while Group B (n = 20) received chemotherapy alone. The primary endpoint was the pCR rate, assessed using the Residual Cancer Burden (RCB) system.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. The pCR rate (RCB-0) was 10% in both groups. The distribution of residual tumor burden across RCB classes was similar, with no statistically significant differences (p &gt; 0.05). Combined chemo-hormonal therapy demonstrated acceptable tolerability; no serious adverse events were reported. All patients completed treatment and subsequently underwent surgery. The addition of tamoxifen did not improve the likelihood of achieving pCR, confirming its limited role in this neoadjuvant regimen.</p></sec><sec><title>Conclusion</title><p>Conclusion. The study demonstrated comparable efficacy and safety between the two treatment strategies.
