Боевые повреждения магистральных сосудов конечностей представляют угрозу для жизни и  сохранения конечности. Традиционный приоритет восстановления артериального кровотока недооценивает роль венозного компонента.</p></sec><sec><title>Материал и  методы</title><p>Материал и  методы. Приведено клиническое наблюдение раненого 22 лет. Множественные осколочные слепые ранения правого бедра вызвали острую ишемию правой нижней конечности III степени.</p></sec><sec><title>Результат и обсуждение</title><p>Результат и обсуждение. Интраоперационно выявлен разрыв подколенной вены с диастазом культей 4 см при сохранной подколенной артерии. Произведено аутовенозное протезирование подколенной вены резецированным фрагментом малой подкожной вены (4 см, диаметр 1 см). Через 30 секунд после восстановления венозного оттока зарегистрированы пульсация подколенной артерии и магистральный кровоток по берцовым артериям. Операция дополнена фасциотомией голени. Послеоперационный период без осложнений. Случай демонстрирует, что выраженное нарушение венозного оттока (венозная гипертензия, стаз) способно клинически имитировать артериальную окклюзию с картиной критической ишемии. Восстановление венозного кровотока устранило ишемию без прямой артериальной реконструкции.</p></sec><sec><title>Заключение</title><p>Заключение. Изолированное повреждение подколенной вены может приводить к критической ишемии вследствие венозного застоя.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Combat-related injuries to major limb vessels pose a threat to both life and limb. Although clinical management traditionally prioritizes restoration of arterial inflow, the contribution of venous injury to limb ischemia is often underestimated.</p></sec><sec><title>Material and methods</title><p>Material and methods. We describe the case of a 22-yearold serviceman with multiple blind shrapnel wounds of the right thigh that resulted in grade III acute ischemia of the right lower extremity.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. Intraoperative examination identified a popliteal vein rupture with a 4-cm gap between the stumps, while the popliteal artery remained intact. Autologous venous reconstruction of the popliteal vein was performed using a resected small saphenous vein graft (with a length of 4 cm and a diameter of 1 cm). Within 30 s of restoring venous outflow, pulsation of the popliteal artery and antegrade flow in the tibial arteries were recorded. The procedure was supplemented with a fasciotomy of the lower leg. The postoperative course was uneventful. This case demonstrates that severe impairment of venous drainage, leading to venous hypertension and stasis, may clinically mimic arterial occlusion and present as critical ischemia. Restoration of venous outflow resolved the ischemia without the need for arterial reconstruction.</p></sec><sec><title>Conclusion</title><p>Conclusion.
