Миома матки является наиболее часто встречающимся новообразованием женской репродуктивной системы. Примерно у 25% женщин миома матки проявляется симптоматически. Частота сочетания миомы матки и беременности варьирует от 1,6 до 10,7%, причем чаще миома встречается у беременных позднего репродуктивного возраста. Традиционно миомэктомию во время кесарева сечения производить не рекомендовалось в связи с риском кровотечения и послеоперационных осложнений. С целью оценки безопасности и целесообразности одномоментной миомэктомии при кесаревом сечении было проведено данное исследование.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. С 2010 по 2015 г. были обследованы 260 беременных с миомой матки, которым произведено кесарево сечение с одномоментной миомэктомией. Данные, полученные при обследовании, сравнивались с данными 96 женщин из группы контроля, не имеющих миомы матки. Исследованы особенности одномоментной миомэктомии при кесаревом сечении в сравнении с проведением кесарева сечения без миомэктомии: оценка интраоперационной кровопотери, пред- и послеоперационных уровней гемоглобина и длительности госпитализации.</p></sec><sec><title>Результаты</title><p>Результаты. Статистически значимые отличия в объеме интраоперационной кровопотери, изменении уровня гемоглобина в результате оперативного вмешательства и в длительности госпитализации между обследованными группами женщин не выявлены. Разработана прогностическая модель для оценки факторов риска интраоперационной кровопотери более 1000 мл.</p></sec><sec><title>Заключение</title><p>Заключение. Размер, локализация миоматозных узлов и их расположение в нижнем сегменте матки не являются факторами риска большой кровопотери при абдоминальном родоразрешении с одномоментной миомэктомией. Фактором риска кровопотери является множественная миома матки, а также размер миоматозных узлов более 5 см. Исследование показало, что одномоментная миомэктомия при кесаревом сечении является эффективной и может выполняться при наличии опыта и квалификации у хирурга. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Myoma of the uterus is the most common neoplasm of the female reproductive system. The frequency of combination of uterine fibroids and pregnancy varies from 1.6% to 10.7%, often fibroids occur in pregnant women of late reproductive age. Traditionally, myomectomy during caesarean section was not recommended because of the risk of bleeding and postoperative complications. To assess the safety and feasibility of a one-stage myomectomy in a caesarean section, this study was conducted.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. From 2010 to 2015, we examined 260 pregnant women with uterine myoma, who had a cesarean section with a single-stage myomectomy. The data obtained during the survey were compared with the data of 96 women from the control group. We study assessment of intraoperative blood loss, pre- and postoperative hemoglobin levels, duration of hospitalization.</p></sec><sec><title>Results</title><p>Results. Statistically significant differences in intraoperative blood loss, changes in hemoglobin levels and in the duration of hospitalization between the examined groups of women were not revealed. A prognostic model has been developed to assess risk factors for intraoperative blood loss of more than 1000 ml.</p></sec><sec><title>Conclusion</title><p>Conclusion. The size, localization of myomatous nodes are not risk factors for high blood loss in abdominal delivery with a one-stage myomectomy. The factor of risk of hemorrhage is multiple uterine fibroids, whereas the size of myomatous nodes less than 5 cm can be regarded as a protective factor for blood loss.
