Увеличившееся количество операций кесарева сечения несет определенные риски при планировании и вынашивании последующих беременностей. Несмотря на то что тенденция к оперативным родам постепенно заменяется естественными, частота повторного кесарева сечения преобладает, причинами которого являются несостоятельность рубца и наличие «классического» (корпорального) рубца на матке. Использование корпорального доступа на сегодняшний день постепенно замещается менее травматичными доступами. Рассмотрение операции «медленного» кесарева сечения как нового подхода к оперативному родоразрешению.</p></sec><sec><title>Цели и задачи</title><p>Цели и задачи. Проведение операции «медленного» кесарева сечения при наличии корпорального рубца на матке.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Приведен клинический случай неполного разрыва матки по корпоральному рубцу при второй беременности на сроке 38 недель. Корпоральный рубец возник в результате проведения операции кесарева сечения при прерывании первой беременности на сроке 22–23 недели.</p></sec><sec><title>Результаты</title><p>Результаты. Пациентка выписана в удовлетворительном состоянии на 5-е сутки домой с ребенком. Послеоперационный период без осложнений, результаты проведенных анализов перед выпиской в пределах нормы. Обсуждение. Проведенный корпоральный разрез при первой беременности был нецелесообразен и нес повышенные риски при последующих беременностях. Для прерывания беременности во II триместре необходимо было рассмотреть терапевтические методы, операцию малого кесарева сечения или кесарево сечение в нижнем сегменте матки.</p></sec><sec><title>Заключение</title><p>Заключение. Проведение операции «медленного» кесарева сечения имеет преимущества по сравнению с классическим кесаревым сечением: мягкая адаптация плода к условиям окружающей среды, очищение слизистой ротоглотки плода, установление более прочной связи между матерью и родившимся ребенком в последующем. В дальнейшем мы планируем провести клиническую оценку детей, рожденных путем «медленного» кесарева сечения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Increasing the initial number of caesarean delivery operations involves certain risks when planning and carrying subsequent pregnancies. Despite the trend towards operative childbirth being gradually being replaced by natural, the frequency of recurrent caesarean is higher than the prevailing rate due to scar inconsistency and the presence of a “classic” (corporal) scar on the uterus. The use of corporal access to date is gradually being replaced by less traumatic access. The operation of “slow” caesarean section is considered as an alternative approach to surgical delivery.</p></sec><sec><title>Purpose and objectives</title><p>Purpose and objectives. To analyse the operation of “slow” caesarean section in the presence of corporal scars on the uterus.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The resulted clinical case of incomplete rupture of the uterus by corporal scar in the second pregnancy at a period of 38 weeks. Corporal scarring as a result of caesarean operations in the case of interruption of first pregnancy at a period of 22–23 weeks.</p></sec><sec><title>Results</title><p>Results. The patient was discharged in a satisfactory condition on the 5th day with the child. Postoperative period without complications; results of analyses performed prior to discharge within normal limits.</p></sec><sec><title>Discussion</title><p>Discussion. The conducted corporal incision at the first pregnancy was inexpedient due to presenting increased risks in subsequent pregnancies. For the termination of pregnancy in the second trimester, it was necessary to consider therapeutic methods, the operations of a minor caesarean section or a caesarean section in the lower segment of the uterus.</p></sec><sec><title>Conclusion</title><p>Conclusion. Conducting “slow” caesarean section has the following advantages over classic caesarean section: soft adaptation of the foetus to environmental conditions, purification of the mucous oropharynx of the foetus, establishing a stronger bond between the mother and her future child.
