Компьютерная томография (КТ) всего тела стала стандартной практикой во многих травмцентрах для диагностики пациентов с политравмой. Цель: оценка диагностического потенциала КТ всего тела для пострадавших с политравмой.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В период с января 2008 по декабрь 2018 г. все гемодинамически стабильные пациенты (сокращенная шкала повреждений (AIS) 3 или более; показатель тяжести травмы (ISS) 15 или более), поступившие в главный Республиканский головной травмцентр ГКБ № 21 г. Уфы с повреждениями двух или более анатомических областей, были обследованы с помощью КТ (голова; шея (включая шейный отдел позвоночника); грудная клетка (включая грудной отдел позвоночника); брюшная полость/таз (включая пояснично-крестцовый отдел позвоночника)).</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. В период с января 2008 по декабрь 2018 г. было проведено 1498 КТ-исследований, из которых в 1368 случаях были диагностированы политравмы (в среднем 2 случая в неделю); 143 (10 % из исследуемой группы) пациента имели травмы только одной области тела. Только у 17 пациентов не получилось завершить обследование на политравму из-за ухудшения состояния, и все они были немедленно возвращены в реанимационное отделение.</p></sec><sec><title>Заключение</title><p>Заключение. За 10-летний период в крупной больнице с неотложной медицинской помощью 1368 пациентов соответствовали установленным критериям для немедленного КТ-исследования в целях диагностики политравмы. Быстро, точно и безопасно был продемонстрирован широкий спектр значительных травм, в том числе 31 перелом шейного отдела позвоночника и 56 пневмотораксов, не обнаруженных на обычных рентгенограммах.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. A computed tomography (CT) scan of the entire body has become a standard practice for the diagnosis of patients with polytrauma in many trauma centres. Purpose: evaluation of diagnostic potential of full-body CT for patients with polytrauma.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. In the period from January 2008 to December 2018, all hemodynamically stable patients (scoring 3 and above in the Abbreviated Injury Scale (AIS) and 15 and above in the Injury Severity Score (ISS)) admitted to the Main Republican Head Injury Centre CCH №21 in Ufa with injuries in two or more anatomical areas have undergone CT scanning of head, neck (including cervical spine), chest (including thoracic spine), abdominal cavity/pelvis (including lumbar spine).</p></sec><sec><title>Results and discussion</title><p>Results and discussion. In the period from January 2008 to December 2018, 1498 CT examinations were conducted. Out of these examinations, 1368 and 143 cases were polytrauma — (on average 2 cases per week) and monotrauma (10 % of the study group) patients. Only 17 patients failed to complete the examination for polytrauma due to deteriorating status. All of these 17 had been returned to the intensive care unit without delay.</p></sec><sec><title>Conclusion</title><p>Conclusion. In a decade 1368 patients met the established criteria for an immediate CT scan for the diagnosis of polytrauma at a large hospital providing emergency healthcare. A broad range of significant injuries was diagnosed quickly, accurately and safely.
