По данным исследования  CREST  (The Carotid Revascularization Endarterectomy versus Stenting Trial), рестеноз в стенте составил 0–6 %, что является достаточно маленьким показателем, учитывая большое количество пациентов, взятых для исследования. В эндоваскулярной хирургии сонных артерий проблема рестеноза до сих пор до конца не изучена. Относительно недавно начали активно применять методы внутрисосудистой визуализации и коррекции рестенозов с помощью баллонов с лекарственным покрытием, повторной имплантации стента или хирургической реконструкции. В некоторых случаях не удается добиться желаемого результата при использовании баллонов с лекарственным покрытием, так как рестенозированный участок не всегда поддается адекватной ангиопластике ввиду выраженной ригидности гиперплазированной неоинтимы. Хирургическая реконструкция также имеет ряд недостатков, в первую очередь сложность удаления самой конструкции стента и невозможность проведения данной процедуры у пациентов высокого хирургического риска.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В нашей статье представлен клинический случай применения stent-in-stent методики коррекции рестеноза баллонорасширяемым стентом с лекарственным покрытием правой внутренней сонной артерии с оценкой отдаленного результата лечения с помощью оптической когерентной томографии.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. Данная тактика лечения нами была принята в связи с наличием гемодинамически и клинически значимого рестеноза ВСА, отказа пациента от КЭАЭ и отсутствием должного эффекта от баллонной ангиопластики. Мы подошли осознанно к выполнению коррекции рестеноза таким способом, исходя из проведенных проб на потенциальную деформацию стента, которые показали отсутствие деформирующих факторов воздействия на рестенозированный участок ВСА. Применение внутрисосудистых методов визуализации значительно расширяет наши возможности в понимании и оценке эндоваскулярных событий.</p></sec><sec><title>Заключение</title><p>Заключение. На наш взгляд, клинически значимые рестенозы в ранее стентированных каротидных артериях требует дальнейшего изучения и представленный клинический случай является единичным решением данной проблемы.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Carotid artery restenosis is a rare complication of carotid stenting. The Carotid Revascularization Endarterectomy versus Stenting Trial (CREST) reveals an in-stent restenosis rate of 0–6 %, a fairly low value given an extensive study sampling of patients. Restenosis still lacks an adequate explanation in endovascular carotid surgery. Intravascular ultrasound visualisation, drug-coated balloons, stent reimplantation or reconstructive surgery have actively been used since relatively recently to tackle restenosis. Drug-coated balloons may fail in certain cases due to hampered restenosis angioplasty in a markedly rigid neointimal hyperplasia. Surgical reconstruction also possessed drawbacks, mostly due to obstacles in the stent removal and the procedure infeasibility in high-risk surgical patients.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The article describes a clinical case of stent-in-stent restenosis correction with drug-coated balloon-expandable re-stenting of right internal carotid artery and a long-term prognosis estimation with optical coherence tomography.</p></sec><sec><title>Results and discussions</title><p>Results and discussions. This tactic was adopted due to haemodynamically and clinically significant internal carotid artery restenosis, the patient’s denial of carotid endarterectomy and insufficiently effective balloon angioplasty. The choice of the correction technique was conclusive basing on a negative stent deformation testing that showed the lack of deforming stress factors at internal carotid artery restenosis.
