Существующие методы лечения показывают неоднозначные результаты эффективности и вызывают многочисленные споры. Данная статья представляет обзор современного состояния проблемы лечения перитонеальных метастазов различного происхождения. В обзоре рассмотрены исторические предпосылки возникновения применяемых методов лечения. Циторедуктивная хирургия и гипертермическая внутрибрюшинная химиотерапия являются предпочитаемым лечением, поскольку при их применении были достигнуты лучшие результаты выживаемости. Альтернативой служат паллиативная хирургия и системная химиотерапия, которые оказались еще более агрессивными и малоэффективными при сравнительной оценке выживаемости. Приведены результаты многочисленных исследований и мнения экспертов об эффективности и целесообразности циторедуктивной хирургии и гипертермической внутрибрюшинной химиотерапии при лечении перитонеального канцероматоза. Однако сегодня рутинное применение данного подхода в повседневной практике активно обсуждается. Несмотря на значительные успехи в лечении пациентов с перитонеальными метастазами, остается целый ряд нерешенных вопросов Спорные результаты исследований, поздняя диагностика, низкая эффективность лечения у пациентов с вы- раженной перитонеальной диссеминацией, возможность проведения циторедуктивной хирургии и гипертермической внутрибрюшинной химиотерапии лишь у ограниченной части больных говорят о важности поиска и разработки методов ранней высокоспецифичной диагностики и тщательного отбора пациентов при выборе той или иной тактики лечения. Знание фундаментальных основ злокачественного распространения является мощным инструментом для определения тактики ведения пациентов и разработки инновационных методов профилактики и лечения канцероматоза брюшины.</p></abstract><trans-abstract xml:lang="en"><p>Peritoneal carcinomatosis is viewed by many experts as a terminal illness with an unfavourable course and prognosis. Existing therapies are controversial and exhibit ambiguous efficacy. We review the current state of the art in therapy for peritoneal metastases of various origin and its historical background. Cytoreductive surgery and hyperthermic intraperitoneal chemotherapy compound the treatment of choice as achieving the highest survival rates. Palliative surgery and systemic chemotherapy are an alternative that proved even more aggressive and ineffective in comparative survival evaluations. Manifold studies and expert opinions exist on the efficacy and expedience of cytoreductive surgery and hyperthermic intraperitoneal chemotherapy in treatment of peritoneal carcinomatosis. Today, however, their routine use in everyday practice is hotly debated. Despite an evident progress in managing peritoneal metastases, a series of questions remain unsolved. Contentious research data, late diagnosis, low treatment efficacy in severe peritoneal dissemination, a limited applicability of cytoreductive surgery and hyperthermic intraperitoneal chemotherapy, all highlight the importance of inventing and developing novel methods for early high-accuracy diagnosis and careful selection of the treatment strategy.
