Рак шейки матки является одним из наиболее часто диагностируемых злокачественных новообразований и четвертой ведущей причиной смерти от онкологии у женщин во всем мире. Пятилетняя выживаемость при местнораспространенном раке шейки матки составляет 91,5 %, в случае с отдаленными метастазами — всего 17,2 %. Метастазы в головной мозг от первичного рака шейки матки очень редки. Освещение и анализ довольно редких клинических примеров могут пролить некоторый свет на решение данной проблемы, помочь сформулировать в дальнейшем тактику лечебно-диагностических мероприятий.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В статье описан случай метастазирования рака шейки матки в головной мозг. Лечение пациентки проводилось по современной методике 3D-конформного облучения на высокоэнергетическом линейном ускорителе электронов Elekta Synergy, модулированной по интенсивности (IMRT) и визуально-управляемой (IGRT) лучевой терапии.</p></sec><sec><title>Результаты</title><p>Результаты. К моменту завершения лечения достигнут клинический эффект. С момента постановки первичного диагноза «рак шейки матки» временной промежуток составил 13 месяцев, с момента постановки диагноза метастазов в головной мозг — 7 месяцев.</p></sec><sec><title>Обсуждение</title><p>Обсуждение. Показатель выживаемости пациентов с метастазами в головной мозг предельно низкий и зависит от возраста пациента, статуса первичной опухоли, наличия экстракраниальных метастазов, а также объема, количества и места локализации метастазов в паренхиме головного мозга. Комплексный подход, включающий хирургический метод, лучевую и химиотерапию, считается лучшим вариантом для повышения выживаемости и улучшения качества жизни пациентов.</p></sec><sec><title>Выводы</title><p>Выводы. Несмотря на доступность всех вариантов лечения, средняя продолжительность выживаемости при выявлении интракраниальных метастазов остается короткой. Исходя из этого, целесообразен поиск и идентификация биомаркеров, обеспечивающих предикцию рецидивирования и метастазирования рака шейки матки. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Cervical cancer is among the commonest malignancies and a top fourth leading cause of cancer death in women worldwide. The five-year survival rate in locally advanced cervical cancer is 91.5%, and only 17.2% — in distant metastasis. Primary cervical cancer metastasis to brain is very rare. Report and analysis of quite rare clinical cases may shed light on this issue, helping formulate relevant therapeutic and diagnostic interventions.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The article describes a case of cervical cancer metastasis to brain. The patient received modern 3D conformal intensity-modulated (IMRT) and image-guided (IGRT) radiation therapies on an Elekta Synergy highenergy linear digital accelerator instrument.</p></sec><sec><title>Results</title><p>Results. Clinical effect has been achieved by end of treatment. Time since diagnosis of primary cervical cancer was 13 months, and 7 months — since diagnosis of brain metastasis.</p></sec><sec><title>Discussion</title><p>Discussion. The survival rate in brain metastasis is marginal-low and depends on the patient’s age, primary tumour state, presence of extracranial metastases, as well as volume, number and location of metastases in brain parenchyma. An integrated approach including surgery, radiation and chemotherapy is considered superior to improve survival and the quality of life.</p></sec><sec><title>Conclusion</title><p>Conclusion. Despite sheer coverage of therapies available, the mean survival rate in intracranial metastasis remains subtle.
