Одним из хирургических методов лечения патологии аортального клапана (АК) является неокуспидализация по Ozaki (AVNeo). Так, по данным последнего систематического обзора свобода от реоперации в течение 1, 3, 5 лет после AVNeo составила 98, 97 и 96 % соответственно. В доступной литературе имеются единичные описания случаев повторных вмешательств после AVNeo. Цель исследования: проанализировать непосредственные результаты повторных вмешательств на АК после AVNeo. Материал и методы. В среднесрочном периоде 11 (4,3 %) больным понадобились повторные операции после AVNeo. Возраст больных составил от 26 до 69 лет. Из 11 прооперированных — 6 мужчин и 5 женщин. У четырех пациентов имелся III–IV класс ХСН по NYHA. Трое пациентов ранее были оперированы по поводу инфекционного эндокардита (ИЭ), а пятеро больных имели двухстворчатый АК. Длительность наблюдения от операции AVNeo до повторных вмешательств составила от 3 до 43 месяцев. Результатыиобсуждение. Всем больным выполнялось изолированное вмешательство на АК. Основные причины дисфункции: ИЭ (6 больных) и отрыв створок в области комиссур (5 пациентов). Семерым больным было выполнено протезирование АК механическими протезами, одному больному выполнено протезирование аортальным гомографтом, одному больному имплантирован биологический протез и двум пациентам выполнена пластика АК. В раннем послеоперационном периоде у одного пациента возникло кровотечение с развитием тампонады и у одного пациента — геморрагический инсульт, ни у одного пациента не было ОПН, периоперационного повреждения миокарда, стернальной инфекции, имплантации постоянного электрокардиостимулятора. В госпитальном периоде умерло двое пациентов. Выводы: повторные вмешательства на АК связаны с высокой частотой осложнений и летального исхода, поэтому профилактика ИЭ и укрепление комиссур должны снизить частоту повторных вмешательств, что необходимо подтвердить проспективными исследованиями на больших когортах больных.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. One of surgical methods for treatment of aortic valve (AV) pathology is Aortic Valve Neocuspidalization Ozaki procedure (AVNeo). Thus, according to the latest systematic review, freedom from reoperation within one, three, five years after AVNeo was 98%, 97% and 96%. Available references provide few descriptions of reoperations after AVNeo. Aim. To analyze the immediate results of AV reinterventions after AVNeo. Materials and methods. In the medium term, reinterventions after AVNeo were necessary for 11 patients (4.3%). The age of patients ranged from 26 to 69 years. 11 patients who underwent surgery included 6 males and 5 females. Four patients had class III-IV CHF, according to NYHA classification. Three patients had previously been operated on for infective endocarditis (IE), and five patients had bicuspid aortic valve. The duration of follow-up, from AVNeo surgery to reinterventions, ranged from 3 to 43 months. Results and discussion. All patients underwent an isolated AV intervention. The main causes of dysfunction were IE (six patients) and leaflet rupture in the commissural area (five patients). Seven patients underwent aortic valve replacement with mechanical valves, one patient — with aortic homograft valve, one patient — with tissue valve and two patients underwent valve repair. In the early postoperative period, one patient had bleeding with the development of cardiac tamponade and one patient had a haemorrhagic stroke. No patients had ARF, perioperative myocardial damage, sternal infection or permanent pacemaker implantation. Two patients died during the hospital period. Conclusions. AV reinterventions are associated with a high incidence of complications and deaths.
