Одним из отечественных механических протезов является «МедИнж-2». Имеются обнадеживающие гемо- и гидродинамические характеристики данного протеза, однако есть лишь единичные исследования, оценивающие результаты данного протеза, имплантируемого в аортальную позицию, и максимальный срок наблюдения в этих исследованиях составляет не более пяти лет.</p></sec><sec><title>Цель</title><p>Цель: оценить непосредственные и отдаленные результаты изолированного протезирования АК механическим протезом «МедИнж-2».</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование включены данные, собранные с апреля 2009 по декабрь 2019 года. За этот период в ФГБУ «Федеральный центр сердечно-сосудистой хирургии» Минздрава России 494 пациентам был имплантирован механический протез «МедИнж-2» в аортальную позицию. Критерии включения: возраст пациентов 18 лет и старше и имплантация механического протеза «МедИнж-2» в аортальную позицию. Критерии исключения: использование других видов механических протезов, комбинированные хирургические вмешательства на сердце. После применения критериев включения и исключения для исследования были отобраны 224 пациента. Медиана возраста включенных пациентов: 55 (50–59) лет. Процент мужчин среди прооперированных: 61,6 % (128 пациентов). Большинство пациентов (62,5 %, или 140 человек) имели III–IV ФК по NYHA. Нарушение ритма в виде фибрилляции предсердий (ФП) наблюдалось у 11,6 % (26 пациентов). Медиана периода наблюдения составила 48 (20–80) месяцев.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. В раннем послеоперационном периоде умерло 3 пациентов (1,3 %), периоперационное повреждение миокарда было зафиксировано в 0,9 % случаев. Имплантация постоянного электрокардиостимулятора составила 2,7 %. Частота инсультов и острого почечного повреждения, требующего гемодиализа, — 0,4 и 0,9 % соответственно. Одногодичная, пятилетняя и десятилетняя выживаемость пациентов: 98, 89 и 84 % соответственно. Одногодичная, пятилетняя и десятилетняя свобода от реопераций на АК: 100, 97 и 96 % соответственно. Кроме того, десятилетняя свобода от тромбоза протеза достигла 98 %.</p></sec><sec><title>Заключение</title><p>Заключение. Клинические результаты и эхокардиографические данные механического протеза «МедИнж-2», имплантированного в аортальную позицию, подтверждают его высокую эффективность в коррекции патологии АК.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. MedInzh-2 is one of Russian mechanical prostheses. Although promising hemodynamic and hydrodynamic characteristics of this prosthesis are reported, only a few studies are available evaluating the implantation of this prosthesis in the aortic position and the maximum follow-up period in these studies does not exceed five years.</p></sec><sec><title>Aim</title><p>Aim. To evaluate the immediate and long-term results of isolated aortic valve replacement with a MedInzh-2 mechanical prosthesis.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included data collected from April 2009 to December 2019. During this period, 494 patients were implanted with a MedInzh-2 mechanical prosthesis in the aortic position at the Federal Center for Cardiovascular Surgery. The inclusion criteria were as follows: patient age of 18 years and older and implantation of a MedInzh-2 mechanical prosthesis in the aortic position. The exclusion criteria included the use of other types of mechanical prostheses and combined cardiac surgery. After applying inclusion and exclusion criteria, 224 patients were selected for the study. Th e median age of included patients was 55 (50–59) years. Among the patients who underwent surgery, the percentage of males amounted to 61.6 % (128 patients). The majority of patients (62.5 % or 140 people) were in NYHA class III–IV. Rhythm disturbance in the form of atrial fibrillation (AF) was observed in 11.6 % (26 patients). Th e median follow-up period was 48 (20–80) months.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. In the early postoperative period, three patients died (1.3 %), and perioperative myocardial damage was recorded in 0.9% of cases. Permanent pacemakers were implanted in 2.7 % of cases. The frequency of stroke and acute kidney injury requiring hemodialysis amounted to 0.4 and 0.9 %, respectively. One-year, five-year, and ten-year patient survival rates were 98, 89, and 84 %, respectively. One year, five-year, and ten-year freedom from aortic valve reoperations were 100, 97, and 96 %, respectively. In addition, the ten-year freedom from prosthetic thrombosis reached 98 %.</p></sec><sec><title>Conclusion</title><p>Conclusion.
